A simple spread sheet is all you need - 5 columns , date, time, stystolic, diastolic and  pulse  rate. In my case a a diabetic i also have a column for sugar level.  I can send you a copy of the form I use if you want.

Hi

I wonder if you could please help me.

We have been asked to record my 92 yr old mum's blood pressure readings at
home and to present the results in spreadsheet form to our GP.

Can anyone help with a Libreoffice template to help record these daily
readings,please?

Many thanks
Srena
